Physical Impressions
We accept traditional impressions, stone models, and physical bites for all restorative categories. Our intake team logs physical cases within 24 hours of arrival to maintain your scheduled seat dates. Follow this checklist to ensure safe transit and precise case entry.
1. Packaging & Prep
- Sanitize & Seal: Place all impressions in moisture-proof bags before boxing.
- Wrap Models: Stone models should be dry and wrapped individually in generous bubble wrap to prevent fractures.
- Secure Box: Use a rigid shipping box with at least 2 inches of padding material on all sides.
2. Core Documentation
- Printed RX Form: Include a completed and signed prescription form with clear clinical notes.
- Clinical Photos: Enclose printed clinical views or note on the RX if photos were sent via email.
- Bite & Records: Securely package any physical bite registrations or previous restorations.
3. Labeling & Shipping

Patient Labeling: Clearly mark the patient name, tooth number and any addiotional notes needed to fulfill RX. Tracking is highly recommended for all shipments.

Submit with Printed RX
For cases requiring physical records or hand-written clinical instructions, download and print our standard laboratory prescription form.
- Fill out restoration details, patient demographics, and tooth shade by hand.
- Submitting via Digital form: fill out all restoration details and additional notes for quality prosthesis
- Submitting via Shipping: Include the paper copy inside your shipment box with physical models or impressions.
